Klothes for Kids Registration
Before you proceed, please take time to gather the following for each child:
1. Social Security Number
2. Grade & school they will be attending for the 2009-2010 school year
3. Shirt, pant, dress, and shoe sizes
You will need adobe reader to view the registration form. If you do not have this program, click here to download it for free.

You are about to start the form. Please read these instructions carefully before clicking the link below: 1. (If an error message "operation not permitted" pops up, click ok). The form should still work. 2. Once you open the form, look near the top of the page. You'll see a check-box next to "highlight fields" and "highlight required fields." Check these boxes and all the fields that you need to fill in will be highlighted. Then click "hide." 3. Start with Parent Name: Use the tab key on your keyboard to move from field to field in the form 4. If you see a yellow bubble, point to it with your mouse and it will provide more instructions. 5. Use the "Submit by E-mail" BUTTON AT THE BOTTOM OF THE FORM to send the form to us. If you have Internet-based e-mail, please be sure to follow the instructions carefully as you are prompted. For example, if you have e-mail through Yahoo, you'll be asked to save the form to your desktop. Then open your e-mail account and send the file as an attachment to k4k@pcrmission.org
