The Life Transformation Program is designed to give an individual the resources needed to become self sufficient in the community.  We provide access to Mental Health Counseling, Case Management, Assistance in obtaining documents needed for employment, Transportation to and from all appointments and work, Assistance in obtaining employment and finding affordable housing.
